How to organize thousands of channels?

My provider gives me like 15,000 live channels and 50,000 VOD movies. It's overwhelming. My TV guide takes forever to load and half the groups are things like '24/7 Friends' or international channels I'll never watch. What's the best strategy for cleaning this up?
